Key Clinical Points Anal canal cancer is a rare cancer with an annual incidence of approximately 8,500 new cases (American Cancer Society, 2018). Several studies have evaluated various treatment regimens for the definitive care of patients with nonmetastatic squamous cell anal cancer. In an analysis of radiation planning quality, 81% of submitted cases required revision of planning following the initial submission secondary to incorrect contouring, noncompliance of normal tissue constraints, or incorrect target dosing. Anal cancer is a radiosensitive tumor with studies of radiation alone associated with 60 to 90% local control rates depending on the size of the tumor (Newman G et al, 1992; Touboul et al, 1994). The initial studies demonstrating the effectiveness of chemoradiation employed doses of 30 Gy in 15 fractions with concurrent chemotherapy and demonstrated greater than 80 to 90% response rates. For example, in a retrospective study of 141 patients with high-risk T1 bladder cancer, radiation alone or combined with chemotherapy was found to be a reasonable alternative to intravesical treatment or early cystectomy. For an individual with muscle-invasive bladder cancer, treatment options include cystectomy or definitive chemoradiation as part of a bladder-preserving approach (Gakis, 2013). In the preoperative setting, there remains insufficient data to determine the benefit of radiation therapy. No evidence of visceral metastases or bulky regional lymph nodes greater than 3 cm on imaging performed within the past 30 days C. Who has received and exhausted all medical or surgical-ablative hormonal treatments. Pain relief is typically achieved 1 to 4 weeks after treatment and the duration of response is 12 to 24 weeks. Pain response rates were similar with 8 Gy in 1 fraction compared with 30 Gy in 10 fractions (66% in each group). While retreatment was higher with patients treated with a single fraction (18% vs. It also states that, strong consideration should be given to a single 8 Gy fraction for patients with limited prognosis or with transportation difficulties. Vertebral body resection and radical decompressive surgery with postoperative radiotherapy was found to be superior to radiotherapy alone in the only randomized trial of spinal cord compression conducted to date (Regine et al. Patients with a single site of cord compression and a minimum three-month life expectancy were enrolled. Patients who received surgery plus conventional radiation therapy retained the ability to walk significantly longer (126 days vs. Based on these results, Radium-223 is medically necessary for the treatment of castration resistant prostate cancer with bone metastases but no visceral metastases and is administered intravenously once a month for 6 months. Metastatic spinal cord compression: a randomized trial of direct decompressive surgical resection plus radiotherapy vs. Meta-analysis of dose-fractionation radiotherapy trials for the palliation of painful bone metastases. Note that all lesions present on imaging must be targeted as a single episode of care. A combination of up to 4 resected and unresected lesions that are individually < 4 cm in size Key Clinical Points I. The clinical response rate, degree of response, and duration of response depend on the extent of tumor and the severity of initial neurologic deficits. Therefore, postoperative whole brain radiotherapy can be recommended for individuals who undergo resection of a solitary metastasis and who have controlled extracranial disease. Due to the palliative nature of the treatment, and dose delivered construction of a dose volume histogram is not medically necessary. A single randomized study found a decrease in cognitive decline in patients who were started on memantine compared to observation, (hazard ratio 0. It found a mean decline in the Hopkins Verbal Learning Test of 7% at four months which compared favorably to historical comparison value of 30%. Including that the improved survival seen on 0933 could explain the improvement in neurocognitive decline. Therefore, hippocampal sparing whole brain radiation is considered investigational. Individuals who present with early and extensive distant failure in the brain and those with limited survival are better treated with whole brain radiation therapy. About 40% of individuals will require whole brain radiation within 6 months of initial treatment with radiosurgery. In individuals who do experience further recurrence in the brain following radiosurgery it is critical to risk stratify this cohort to determine who will benefit from further radiosurgery vs. One hundred ninety-four (194) patients were included in the study with a median follow up of 11. Indications for post mastectomy radiotherapy include the presence of multiple positive axillary lymph nodes, positive or narrow margins (< 1 mm), or large primary tumor size (> 5 cm). In some women over the age of 70 who have been diagnosed with invasive breast cancer, radiation therapy may be safely omitted, especially if they have comorbidities. At 10 years, the hypofractionated regimen was not inferior to standard fractionation with respect to recurrence, survival or toxicity. The recently updated evidence-based guideline on radiation therapy for the whole breast has expanded upon the original 2011 recommendations (Smith et al. The guideline now recommends a hypofractionated regimen for all age groups and all stages as long as additional fields are not used to encompass regional lymph nodes. Recommended dose regimens are 4000 cGy in 15 fractions or 4250 cGy in 16 fractions. There is no longer a contraindication to the use of chemotherapy prior to radiation or the use of concurrent treatment with hormonal or trastuzumab. Fifth disease is a benign rash illness of childhood sometimes called erythema infectious. The illness begins with prodromal phase of mild fever with non-specific symptoms of headache, malaise and muscle aches. The rash begins as a red, flushed appearance on the cheeks, giving a "slapped cheek" appearance. The virus can cause stillbirth and fetal hydrops in pregnant women experiencing a primary infection. Please consult your physician if you are pregnant and a child in the childcare facility has fifth disease. Children with fifth disease do not need to be excluded from day care, as they are unlikely to be infectious after the rash appears, and the clinical diagnosis is made. If the test is positive keep your child home until any serious diarrhea or illness is over and your child has received medication. Giardia is a very small (microscopic) parasite that can infect the intestines and stools. The infection, whether or not it causes symptoms, can come and go for months if not treated. Giardia can be diagnosed by a test called "stool culture for ova and parasites", in which the stool is examined under a microscope. Be sure everyone washes their hands carefully after using the bathroom, or helping a baby or child with diapers or toileting, and before preparing or eating food. If someone in your family develops diarrhea, talk to your health care provider about getting a stool test. Medication is recommended for children and adults with Giardia in their stools, as it shortens both the length of the illness and the time the germ is found in the stool. They include a sore throat, runny nose, cough, sneezing, ulcers on the tongue, and blisters on the hands, feet or buttocks. Wash hands immediately after changing diapers, or helping persons with this disease. It is used to stimulate motor activity of the small intestine and colon, as in certain types of nonobstructive paralytic ileus. It is useful in treating atony of the detrusor muscle of the urinary bladder, in myasthenia gravis, and sometimes in glaucoma. Like other cholinesterase inhibitors, neostigmine requires an intact postganglionic innervation for full development of its actions. Edrophonium (Tensilon ) is a quaternary amine widely used as a clinical test for myasthenia gravis. Many phosphorothionates, including parathion and malathion undergo enzymatic oxidation that can greatly enhance anticholinesterase activity. Differences in the hydrolytic and oxidative metabolism in different organisms accounts for the remarkable selectivity of malathion. In mammals, the hydrolytic process in the presence of carboxyesterase leads to inactivation. This normally occurs quite rapidly, whereas oxidation leading to activation is slow. In insects, the opposite is usually the case, and those agents are very potent insecticides. Some patients encounter muscarinic side effects due to the inhibition of peripheral cholinesterase by physostigmine. The most common of these side effects are nausea, pallor, sweating and bradycardia. Several centrally acting drugs produce an acute toxic psychosis characterized by confusion and the peripheral signs of cholinergic blockade. These drugs include several plant toxins, antidepressants, H1 receptor antagonists with central effects, and several antiparkinsonian drugs and antipsychotic drugs. Cholinesterase inhibitors that cross the blood-brain barrier are suitable to reverse the central anticholinergic syndrome. Although physostigmine effectively wakes up such patients briefly, it is not certain that its use results in a long-term better prognosis. Two newer agents donepezil (Aricept ) and rivastigmine (Exelon ) have little hepatotoxicity and have replaced tacrine. On the accompanying tables, the effects of intoxication and the therapeutic approach to treatment are outlined. This drug counteracts cholinesterase inhibitor intoxication by reactivating the cholinesterase enzyme. Pralidoxime combines with the anionic site on the enzyme by electrostatic attraction to the quaternary N atom, which orients the nucleophilic oxime group to react with the electrophilic P atom; the oxime-phosphonate is split off, leaving the regenerated enzyme. War Gases Long-acting or "irreversible" cholinesterase inhibitors (organophosphates) are especially used as insecticides. Cholinesterase inhibitors enhance cholinergic transmission at all cholinergic sites, both nicotinic and muscarinic. Sarin which is a war nerve gas is a binary agent composed of two components that are not toxic until mixed. Nerve gases such as the cholinesterase inhibitor, sarin, have been the chemical weapons of choice for over 50 years. Sarin is an easily dispersed agent that acts extremely quickly when absorbed through the skin or inhaled. The final stage of sarin synthesis usually takes place while the missile or other delivery vessel is in flight because it is safer to store the component reagents than the more dangerous sarin itself. Skeletal Muscle Relaxants Skeletal muscle relaxants fall into two broad categories. The neuromuscular blocking drugs are used to produce muscle paralysis and act at the neuromuscular endplate. The spasmolytic drugs have much milder actions and act at sites other than the muscle endplate. The pharmacology of the neuromuscular blocking drugs is historically very complex, and several lectures in this course were once devoted to it. This no longer seems to be necessary in order to gain the knowledge required to use these agents appropriately. Much of the complexity of these drugs relates to the varying characteristics of the blockade they induced (depolarizing versus nondepolarizing), which seems simpler now that we understand it better. Since skeletal muscle contraction is elicited by nicotinic (Nm) cholinergic mechanisms, it has similarities to nicotinic neurotransmission at the autonomic ganglia. Interestingly, two different kinds of functional blockade may occur at the neuromuscular endplate. One type mechanistically resembles muscarinic blockade, a adrenoreceptor blockade and b -blockade described above, and is called nondepolarizing blockade. The depolarizing type of blockade is elicited by an agonist effect whereby there is stimulation of the nicotinic endplate receptor to depolarize the neuromuscular endplate. This initial depolarization is accompanied by transient twitching of the skeletal muscle. However, with continued agonist effect, the skeletal muscle tone cannot be maintained, and, therefore, this continuous depolarization results in a functional muscle paralysis. It has a comparatively long (60 minutes) half-life, but this can be increased in patients with impaired renal function. Blockade by agents such as tubocurarine, pancuronium, and doxacurium can be reversed by increasing the Page 27 Pharmacology 501 January 10 & 12, 2005 David Robertson, M. It has a shorter half-life (5-10 minutes) and must be given by continuous infusion if prolonged paralysis is required. In practice, succinylcholine is often used to initiate paralysis and paralysis is then continued with a non-depolarizing agent. In patients with pseudocholinesterase deficiency, succinylcholine half-life is greatly prolonged, and such patients may regain control of their skeletal muscles slowly after a surgical procedure. It is obvious that patients with myasthenia gravis would be dangerously sensitive to the effects of neuromuscular blockers, as are patients with certain forms of carcinomatous neuropathy. There is a typical pattern of relaxation of muscles after the administration of an agent such as tubocurarine: extraocular muscles are affected first, then the muscles of the hands and feet, head and neck, abdomen and limbs, and finally the muscles of ventilation. With the administration of neuromuscular blockers, there is often histamine release and this can reduce blood pressure, increase respiratory secretions, and sometimes produce a degree of bronchospasm. Some agents can also stimulate or block sympathetic and parasympathetic effects on various tissues. In practice, some neuromuscular blockers have resulted in very high blood pressures and heart rates in occasional individuals and very low blood pressures and heart rates in others, primarily because of their disparate effects on autonomic ganglia and muscarinic receptors. These include a number of antibiotics (aminoglycosides gentamicin, kanamycin, and streptomycin) and inhaled anesthetics such as isoflurane. Therefore, there is a great need for specific muscle relaxants, which can be used in spastic states associated with trauma, inflammation or psychogenic disorder. They are of two types, (a) those that act directly on muscle and (b) those that act indirectly by depressing nerves. Dantrolene reduces the release of activator calcium from the sarcoplasmic reticulum. Dantrolene is widely used to treat the muscle contractures associated with malignant hyperthermia. It may be effective in relieving spasticity due to cerebrovascular damage, spinal cord lesions, multiple sclerosis or cerebral palsy. It is not useful in the treatment of fibrositis, bursitis, arthritis or acute muscle spasm of Page 28 Pharmacology 501 January 10 & 12, 2005 David Robertson, M. Purchase aldara with visa.
